How to crack MRCS Part A and the Best books for MRCS Part A preparation (Updated)


Sharing my preparation plan and resources for preparation of MRCS Part A here. I took MRCS Part A exam during second year of my MS General Surgery residency. I would rate the difficulty of the exam as moderate, at least it was not something which can be taken lightly. There were times in the exam hall when I was doubting myself over my responses to the questions. Quite a few questions were very tricky, so you have to be on your toes while doing preparation.Until the day of the result I had a doubt over me cracking the exam, but the big day came as a pleasant surprise. 

Below I will break up the preparation into a few steps:


Step 0 : Understand what is MRCS Part A Exam and Which College to choose?

Step 1: Decide date of the exam giving yourselves enough time and a good headstart over the competition

Step 2: Choose your study resources

Step 3: Join various Facebook communities where students and seniors help each other over study resources and important information regarding the exams

Step 4: Revise what you have read

Step 5: Hold onto your nerves in the examination hall and don't do silly mistakes


Step 0: Understand what is MRCS Part A Exam and Which College to choose?

Full form of MRCS is Member of the Royal College of Surgeons. The MRCS examination is a crucial milestone for a surgical career in the UK; it determines whether a surgical trainee possesses the correct knowledge, skills and attributes to complete basic training and to progress to higher levels of specialist surgical training. Since MRCS is an intercollegiate exam, so after passing part A in any of the above college one can give part B in same or any of the other colleges. Degree will be awarded by the college in which Part B is given. There is no difference in value of degree awarded all are equal only difference is Edinburgh is more immigrant friendly college and England is more popular and difficult to get through. MRCS Part A exam is conducted by:

Step 1: Decide date of the exam giving yourselves enough time and a good headstart over the competition

  • Deciding when to give the exam is one of the important factors which will drive your success in the exam
  • It is held three times a year in January, April and September with booking closing approximately 2 months before the exam date.
  • You can follow these two routes
    • Express Preparation: Requires 6 months. Suitable for those who do not have a very tight work schedule and can take out a good relaxed 4-6 hours of time to study each day.
    • Laid Back Preparation: Requires 1 year. Suitable for those who have a very busy work schedule and cannot devote more than 2-3 hours per day of study time.
  • Once you decide about which cohort you would fall in, book a date for the exam
  • Until you don't book a date, your brain will be in a denial about the exam preparation and you will be as lazy as a toad. So if you are really serious, book a date at the earliest.

Step 2: Choose your study resources

  • What to read to crack MRCS again depends on when are you starting your preparation. 
  • In case you are going the express route your resources will include more of MCQs and EMQs, and less of theory books which will be used only for reference
  • In case you choose to go the laid back route, you have a chance to work on your weak areas, strengthen your basics and mix it with a right proportion of MCQ/EMQ practice everyday

I cannot overemphasize the importance of doing as many MCQ/EMQs as possible because our brain usually is good at recalling readymade retained information in pressure situations, as in the exam hall it is sometimes difficult to come at conclusions. One month before the exam stop reading anything new and focus only on the revision. Stick to MCQ/EMQs for your revision. Try to take a glance of all of them atleast once. 

Step 5: Hold onto your nerves in the examination hall and don't do silly mistakes

On the D-Day... 
  • Stay composed
  • Read questions carefully
  • Mark the options as you attempt questions
  • Keep a track of time, remember you have less than a minute to attempt each question. Time is one of the important factor which will determine your success.
  • Avoid silly mistakes
  • Keep your senses working 
  • Don't get nervous
  • Take good breakfast, stay energized and well hydrated 
If you have studied well, you will definitely clear the exam.

Frequently Asked Questions about MRCS Exam preparation


1) What is the best time for appearing in MRCS Part A for Indian doctors?

For MRCS Part A, the best time to prepare and appear in exam in my view is 2nd year, because you have had a good practical exposure to most clinical situations which are asked in the exams by then. This will make reading theory and attempting questions more interesting.


2) Can I appear in MRCS Part A after successfully completing my pg entrance exams?


However if you think you have enough time available after your pg entrance exams, and you think you are comfortable with basic knowledge of  supespecialties like urology, vascular surgery, orthopaedics, neurosurgery etc, you can go ahead with MRCS Part A.

3) What is the best time for appearing in MRCS Part B exams for Indian doctors?


For MRCS Part B, the best time to prepare is 3rd year, you are very strong clinically by then and you will be able to prepare with minimum effort. Your probability to pass the exam will also be higher. In addition to it, it will also help in preparation of final year general surgery clinicals.

4) After doing MRCS part A, how much time we have to pass MRCS part B? 

After passing part A, you are required to pass Part B of the examination within seven years. Otherwise successful MRCS Part A result will be considered as expired.


5) Is MRCS Exam mandatory for doing FRCS?

There can be a direct entry to FRCS too but for experienced surgeons only and that too after satisfying a lot of other criteria. But MRCS makes it easier to get GMC registration, Royal college sponsorship, fellowships and jobs in the UK.


6) Can I start my preparation for MRCS immediately after finishing my housejob?

You can start your preparation for MRCS Part A immediately after finishing your housejob but it is always better to have some basic surgical experience before you appear for MRCS Part B.


7) Is MRCS Degree of any use in India?

MRCS has only academic value in India and showing off a foreign degree on your CV to your employers and to your patients. Maybe it can help you initially in your career in converting your job interviews easily. . It shows that your standard of knowledge matches the international standards. That is the reason many specialists resort to naming the institution or University from which they obtained the MRCS/ FRCS in brackets after the degree. MRCS is a very standardized examination with emphasis on basic sciences. MRCS can authenticate minimum proficiency in the basic principles of General Surgery.

However, there is no significant difference in the packages between MRCS qualified and non-qualified doctors. In the Middle Eastern and other countries, doctors with MRCS are preferred and given higher seniority and packages.

8) What are the changes in the pattern of MRCS Part A exam?

MRCS A exam earlier used a mixture of single best answer multiple choice questions (MCQs) and extended matching MCQs (EMQs) in Paper 2. From September 2018, only single best answer MCQs are being asked in the paper.

Sir, 

Do u think there is any value of doing MRCS???

If one doesn't want to go to uk ???

I dont think I want to settle in uk.

Should I still write MRCS ???


For practical purposes, it only gets your name in the GMC register. NHS needs doctors from overseas to fill certain jobs which are not taken up by local graduates as they do not lead to a consultant post. These are called trust grade jobs or non-training jobs. Few from this cadre may get training jobs leading to CCT, which is a must for becoming a consultant. 

So MRCS is a basic exam that gives you eligibility to start work in NHS at the junior level.

In India, MRCS is just a post-nominal to show off that you have a foreign degree. There is no difference in packages and seniority of MRCS vs Non-MRCS doctors in both corporate and government hospitals.


MRCS has no value after MBBS, as it will not fetch you a job and won't have any academic or "show-off" value as you are not a surgeon. The academic and "show-off" value of MRCS in India exists only if you have MS or DNB General Surgery degree. 


But if you want to practice outside India, it adds lots of value to your CV. In the middle east, for example, MRCS doctors are preferred and given higher packages as well as seniority.

  • To enter the examination, a candidate must possess a primary medical qualification that is acceptable to the United Kingdom General Medical Council for Full or Provisional Registration or to the Medical Council in Ireland for Full or Temporary Registration; overseas candidates must hold a primary medical qualification acceptable to the Councils of the colleges.
  • Prospective candidates who wish to check the acceptability of their medical degrees should see the International Medical Education Directory (IMED) on http://avicenna.ku.dk/database/medicine or the World Directory of Medical Schools on http://search.wdoms.org/. If the medical school does not appear on this list, candidates should contact the examinations department/section at the college to which they wish to apply for the examination for further clarification.

OPPORTUNITIES AFTER MRCS


Full form of FRCS is Fellow of the Royal College of Surgeons


It is a professional qualification required to practise as a surgeon in the United Kingdom. This post nominal can be obtained after working for 6 years in a particular subspecialty that you choose and then qualifying the FRCS exam for the same subspecialty. After obtaining this degree, you will considered a consultant in that particular surgical subspecialty.

This roughly translates to MCh ( Super speciality ) in case of India.

There are two versions of FRCS, one for domestic and other for International candidates.

  • JCIE  FRCS (Joint Committee on Intercollegiate Examinations) (domestic - UK & Ireland) 
  • JSCFE FRCS (Joint Surgical Colleges’ Fellowship Examinations) (international)

JSCFE FRCS (International) can be obtained in following surgical subspecialties:

  1. Cardiothoracic
  2. General Surgery
  3. Neurosurgery
  4. Otolaryngology
  5. Trauma & Orthopaedic
  6. Urology
FRCS for Oral & Maxillofacial Surgery, Paediatric Surgery, Plastic Surgery and Vascular Surgery are not available while you are in India.

FRCS for Oral & Maxillofacial Surgery, Paediatric Surgery, Plastic Surgery and Vascular Surgery are not available while you are in India.

I don't think it will make sense doing FRCS General Surgery after doing MCh Surgical Gastroenterology.

Can I do FRCS after my MBBS? 

No you cannot do FRCS after MBBS. FRCS is a specialist qualification and a person is not eligible to appear for FRCS straight after MBBS. Just wanted to ask 1 doubt.
Myself , i completed MS General surgery and i am working here in india as Asst Prof since past 5 years.
I wish to relocate to UK.
My ultimate aim is to become a consultant there .
However , i could find that there are 2 boards of FRCS and international FRCS is not of much importance when it comes to working in UK . 
Is it true sir?

If relocation to the UK is your priority, clear MRCS A and B, IELTS/ OET, then get GMC registration, get a training job (matching your experience level), and then give FRCS Gen Surgery there.

You can buy Pastest and any one other course on sharing basis between your friends. Pastest has a good coverage and most people use it in their preparation. I used Pastest and eMRCS  which were shared by two of us preparing for MRCS.

I passed MRCS Part A Jan 2020 attempt in first attempt with 80% marks yesterday,I wanted to share my experience as it might help someone.

As for my background, I am a fresh graduate and got my GMC Registration last month,via PLAB pathway. I wasn't planning on applying for jobs in the NHS right away due to some personal circumstances, so I decided to utilize this time period by going for MRCS, as I plan on pursuing surgical training later.

Application :


Application process for MRCS is pretty straight forward. You can apply through either one of the four Royal College of Surgeons.  It doesn't make any difference which college you choose, I went for RCS England,but it is a bit more expensive as compared to the others.

Once you make account on RCS website and apply for upcoming exam, you have to send a verified copy of your degree to RCS by post within a few days.There are three options for verification :

*British Council:   I went to British Council Office Islamabad, but they told me that it is a complicated procedure and would take atleast 2 weeks.

*Any person holding Full GMC Registration can do it too, but I didn't know anyone personally who could do it.

*Third option is the easiest and I would recommend you to go for it. Go to The Notary Public in your area ( I went to Lawyers' Chambers in F-8 Markaz in Islamabad) with your original degree and a copy and they will do it within 5 minutes for Rs 150/-.

And after verification, dispatch the documents to RCS via post. (DHL delivers the documents within 5 days)

Preparation:


I can never prepare from multiple sources, and the biggest difficulty I faced when starting preparation was that everyone was recommending at least 6-7 different sources for preparation.

I have narrowed them down to these four, they are the best and the most widely used:

EMRCS : It is the most important one and the closest to the actual exam. There are around 2800 MCQs in it.

Pastest: Keep it as your second source of study. It has around 2300 MCQs.

Basic Sciences for the MRCS by Andrew T Raftery.

Oxford Handbook of clinical medicine (Only Surgery portion).

The way I prepared was that I would do one topic e.g. Cardiovascular Physiology from Raftery E-Book, and then solve MCQs from both EMRCS and Pastest of the same topic. In this way you can prepare effectively.

For surgery part, do the topic from OHCM ,and then related MCQs from EMRCS and Patest.
I subscribed to both EMRCS and Pastest, and they are  worth it in my opinion. You will be booking the exam for 550£, so 120£ for these resources is understandable. The good thing is that you can reset the questions and start from scratch again. Only 7 attempts are allowed for Part A, so make sure you are well-prepared before you appear in the exam.

PDF versions of these question banks are also available in various groups.

Anatomy videos by Prof. Harold Ellis that come with the Pastest subscription are THE BEST. Make sure to open  pastest on laptop. ( I used the mobile app throughout and came to know about the videos only 2 weeks before the exam 🤦 )

Last but definitely not the least, do Fawzia Sheet (300 most high yield MCQs). I can't stress it enough how vital those are.

Preparation Time:


I prepared in 2 months, but please give yourself atleast 3-4 months to do all the questions atleast 4 times. Remember, revision is the key.

For me,It was the best decision to take this right after PLAB, as the PLAB knowledge helps you immensely, and it is easier to focus solely on preparation as compared to taking the exam later while doing a job in the NHS.

Hello sir

I have recently finished DNB Ortho and i have also cleared MRCS Part A. Planning to go for Part B in 2019. Sir I was wondering if u could tell me about prospects of getting training posts in Singapore. As I hear it is difficult to get one in the UK.

I read on your page 'World Surgery Forum' that one of the countries where MRCS may have recognition is Singapore. But I am unable to find much info about the system etc there

International Medical Graduates can take following routes to practice in Singapore 

Indian Graduates from following Medical Colleges can apply for fellowships directly without giving any exams:
  1. All-India Institute of Medical Sciences, New Delhi 
  2. Christian Medical College, Vellore 
  3. Armed Forces Medical College (AFMC), Pune 
  4. Jawaharlal Institute of Postgraduate Medical Education & Research (JIPMER), Puducherry
  5. Lady Hardinge Medical College, New Delhi
  6. Grant Medical College, Mumbai
  7. St. John’s Medical College, Bangalore
  8. Madras Medical College, Chennai
If you are not from any of these 8 colleges in India, then the next option is having a recognized medical qualification from any of these countries: UK, Ireland, Australia, New Zealand, US and Canada. Thus MRCS is one qualification which you can obtain in India and it makes you eligible to apply for fellowships/ jobs in Singapore.

Singapore is not very conducive for foreign doctors from developing countries to become consultant grade but they have several gaps in the system for which they need doctors from India, Bangladesh, Philippines, Indonesia, etc to work as Medical Officers.

Minimum 5 years of post-housemanship experience

Sir I am doing my final year mbbs now. I wish to give MRCS after completing my internship. Supposedly if I pass both parts of MRCS, on what basis do i get A training post in the UK or Ireland.

After passing both MRCS exams, you will have to pass IELTS and register with GMC. Then you can apply for training and non training posts in the UK. You can apply for training posts under two schemes:

- International Medical Graduate Sponsorship Scheme (IMGSS)
- International Surgical Fellowship Programme

And if i dont get one what am i supposed to do.

You can go for a non training post and keep applying till you get one.

Is there a maximum waiting period after u which u certainly will get the desired post.  

There no such period after which you would be guaranteed a desired post. It will depend on your applications to deaneries, via forms and interviews.
